As part of the Ringette Ontario "Return-To-Play" guidelines, there are several new requirements to ensure the health and safety of all of players, staff and families.
The following forms are required to be completed for each player to ensure the understanding of all the new guidelines as they apply to COVID-19. In addition, all parents, bench staff, family, friends that attend any of the events, practices, off-ice sessions, etc must complete these forms.
A participant is any member of Ringette Association of Burlington. This includes players, coaches and bench staff, parent volunteers for all roles.
One-Time forms need to be completed only once per participant.
Daily Attestation forms need to be completed for each and every event (practice, game, dryland, party, etc). These are to ensure each participant has declared themselves healthy to participate in that particular event, but also act as the required
contact tracing information as required by the regional health authorities.
Do not print off these forms. We are collecting them on-line. Your acknowledgement on the form is your electronic signature.
The following forms are a 'One-Time' requirement.
The participant field should contain the name of the person that the form is being filled out on behalf of.
The following DAILY ATTESTATION form is needed at every event, by every participant entering the facility.
JOB AID FOR COVID SAFETY MANAGERS:
The Cohort Safety Manager must provide their names to RAB Web Admin to be granted Admin Access to the back end of this tracing tool.
FORM SUBMISSION EXAMPLES:
Jane has 2 children who play for RAB.
Jane's two children require the Informed Consent and Assumption of Risk agreement, so she does this:
- Submits once for Kid1, with Kid1's name listed as the participant.
- Submits once for Kid2, with Kid1's name listed as the participant.
Jane does not require that form, because she is over the age of 18.
Jane is an adult and therefore requires the Release of Liability, Waiver of Claims and Indemnity Agreement form, so she does this:
- Submits once for herself, with her own name listed as the participant.
Her children do not require that form, because they are minors.
When completing a form that needs to be done for everyone such as the Declaration of Compliance, she does this:
- Submits one form for Kid1 with their name listed as the participant.
- Submits one form for Kid2 with their name listed as the participant.
- Submits one form for herself with her own name listed as the participant.
Every time Jane or her children go to the arena, she must submit the
Daily Attestation form for each person.
On Sunday she brings them to the arena, so before arriving, she does this:
- Submits one daily attestation form for Kid1 with their name listed as the participant.
- Submits one daily attestation form for Kid2 with their name listed as the participant.
- Submits one daily attestation form for herself with her own name listed as the participant.
On Tuesday, Jane's mom brings the kids to the arena because Jane can't go. So she does this:
- Submits one daily attestation form for Kid1 with their name listed as the participant.
- Submits one daily attestation form for Kid2 with their name listed as the participant.
- Asks her mom to submit one daily attestation form for herself, with her own name listed as the participant.
